The indexing method that got me through the June sitting (method only, no files)
not sharing any files, just how i built it, since this comes up constantly.
one line per entry, always the same shape: term, then page, then nothing else. if i could not compress an entry to a term i did not understand the thing well enough yet and that was the actual signal.
i only added an entry after i had failed to find something. not while reading. that meant the index was a record of my own gaps rather than a summary of the book, and it was maybe 35 pages in the end.
numbers went in, reasoning did not. deadlines, thresholds, limits, all in the index. anything requiring thought stayed on the page where i could read it properly.
tabs were three colours and i wrote what they meant on the inside cover, because in week two i had already forgotten.

The "only add it after you failed to find it" rule is the single best piece of indexing advice on this board and I wish someone had told me in my first cycle.

Adding one thing that took me two attempts to learn. Put the index together yourself even if it is worse than someone else's.
You find things in your own because you remember writing them. Quality matters way less than whether your hands made it.
